primary purpose of position is to provide
intensive community based services for people who have a history of chronic homelessness, who have tri morbid conditions such as mental illness, a substance use disorder and a chronic medical condition, and are high utilizers of the Criminal Justice system. This service is done by working collaboratively with an interdisciplinary team to provide skills training, rehabilitative psychosocial services and service coordination to assist consumers to remain in the community, obtain/maintain housing stability and promote wellness,
while decreasing the need for inpatient hospitalization and/or incarceration
The position will require strong engagement and substance use intervention skills. This position will require effective collaboration with various officials at the mental health court to identify opportunities for diversion. All services should be recovery focused and in line with a Housing First philosophy, helping clients to secure independent housing, competitive employment, and achieve progress in their recovery.

Work is performed under moderate supervision of the unit manager with extensive latitude for the use of initiative and independent judgment. This positionrequires
the transportation of consumers in personal vehicle and on call rotations.
Minimum two years direct client care experience in mental healthrequired.
Current and valid Texas driver's license and a clean driving recordrequired.
Monday - Friday
with some weekend rotation depending on needs of the ACT clients. On-Call schedule overnight, weekends and holidays.
